Look out for a link parents/carers of your reception aged children. This will be coming your way at the beginning of the Spring term, January 2020. The online Health needs assessment is called The Lancaster Model health questionnaire and asks a set of questions to understand the health and wellbeing of your child. From this the Public Health Nurses (5-19) can analyse the responses and offer support to you and your child if needed.

In Year 6, the children complete the online health assessment and consent will have been obtained from the parents/carers beforehand.  Again the results will be analysed and any issues or concerns can be dealt with and support and advice offered to the child. This is happening in Primary Schools during the Autumn term 2019 from September.

Contact your local Public Health Nurse team, 5-19, (School Nursing) for more information if you wish on – 0300 123 7572.
