Introduction to the School Nursing Service
When your child starts school their care is transferred from your local 0-5 Public Health Nursing Team to the 5-19 team. This service is provided by Hertfordshire Community NHS Trust in partnership with the Hertfordshire Family Centre Service.
Your Public Health Nursing team will offer support with:
– Advice on childhood immunisations
– Emotional and mental health issues
– Long term and chronic health issues
– Toileting concerns
– Support for young carers
– Healthy eating and weight management.
School can complete a referral to the school nursing team for your child or see below you can make a self referral.
