The Lancaster Model online Health Questionnaire portal is open now for parents of Reception children.

Parents of children in Reception.

Your school will be sending out information from the School Nurse team in Herts regarding completing The Lancaster Model online health questionnaire for your child.

This will enable the School Nursing team to respond to any identified health need of your child to ensure their needs are being met. On completion of the questionnaire the School Nurse team may contact you to discuss your concerns and offer support and advice in areas that you as parents identify.

The online portal will open on 27th May and close on 30th June 2021.
