If you or your child are aged 14+ and are listed on your GP\u2019s learning disability register, then you qualify for a free annual health check at your doctors. If you are not sure if you\u00a0are\u00a0listed on the learning disability register you can contact your GP to ask.\u00a0<\/p>\n
Why are annual health checks important and what are they?\u00a0<\/p>\n
\tA health check will help you make sure you are doing the right things to stay healthy.\u00a0
\n \tIt\u2019s good to have one even if you do not think you have a health problem.\u00a0
\n \tThey take place at your local doctor\u2019s surgery.\u00a0
\n \tThe doctor or nurse will examine you and ask some questions about your health. You may be asked to take a urine sample with you for testing.\u00a0
\n \tYou can talk to your doctor or nurse if you are worried about anything.\u00a0
\n \tIt helps to get to know your practice and for your practice to better understand and know you.\u00a0
\n \tDo let the surgery know if there\u2019s anything you need to make it easier for you to have a health check. You can take someone with you if you want.\u00a0<\/p>\n
What happens after my health check?\u00a0<\/p>\n
\tYou will be given a health action plan.\u00a0
\n \tYou will be asked to try and do the things that are listed in your action plan to stay healthy.\u00a0
\n \tRemember to ask for advice with your health if you need it. Your doctor or the learning disability nurses can help you with this and you can have medicine reviews with your pharmacist.\u00a0<\/p>\n
