The care navigation team are an administrative team working together with health professionals and external partner agencies to support young people aged 0-19.<\/p>\n

\tDirecting you to local support networks.
\n \tCollating and sharing information with professionals involved to enable a more efficient service.
\n \tProviding confirmation of health appointments detailing the clinic, date, time, and venue.<\/p>\n
The care navigation team may share the following information with health professionals that are currently involved in your care:<\/p>\n

\n \tAppointment information.<\/p>\n
Consent will be gained to share information between professionals.<\/p>\n
They may request further information from, and work with relevant agencies, so that they can make sure that any help needed is made available.<\/p>\n
