Registering with a GP
A General Practitioner (GP) is a doctor who has extra training to work in the community, dealing with all kinds of health problems.
The surgeries or practice they work from can offer a range of health services, including support for mental health, pregnancy related services and minor injuries. GP practices also work closely with other agencies like social care.
When choosing a GP practice to register with, it’s a good idea to look at the range of facilities they offer and see if they will be able to support your needs.
How do I find a GP?
Everyone has the right to register with a GP practice of their choice. However, a GP practice may refuse to register you if:
- It has no capacity to take on new patients
- If you do not live within its practice boundaries

The registration process
To register with a GP practice, you’ll need to formally register with NHS England as an NHS patient by completing the GMS1 form. You can get a copy of this from the practice or you can download it here.
When this form has been completed, NHS England will transfer your medical records to your new GP. The practice will write to you to confirm that you are registered with them.
