Accessibility Statement for Health Forms
This accessibility statement applies to forms.chathealth.nhs.uk
This website is run by Leicestershire Partnership NHS Trust. We want as many people as possible to be able to use this website. For example, that means you should be able to:
- change colours, contrast levels and fonts
- zoom in up to 300% without the text spilling off the screen
- navigate most of the website using just a keyboard
- navigate most of the website using speech recognition software

How accessible this website is
We know some parts of this website are not fully accessible:
- Some radio button/multiple-choice questions cannot be answered using the keyboard
- Action buttons do not respond to changes in text spacing
- Images have not been marked as either meaningful or decorative
- Images do not provide alternative text
- There is no bypass block for keyboard users
- Alert messages are difficult to tab away from using the keyboard
- Some footer text has insufficient contrast between the foreground and background

Some of our pages link to PDF documents provided by external organisations for additional content. Whilst these documents are outside of our control, we will update links to web page content where possible when articles are reviewed.
Live video
We do not plan to add captions to live video streams because live video is exempt from meeting the accessibility regulations.

This website was last tested on 20th October 2021. The test was carried out in-house by Leicestershire Partnership NHS Trust using the Accessibility Insights for Web tool.
