The Immunisation Service offers vaccinations to children and young people aged 4-16 who live in Hampshire and the Isle of Wight.  Vaccination clinics are offered in venues across the county throughout the year.

If you would like your child to be vaccinated or if you wish to decline the vaccine, submit an electronic form.  Use the code SH999999.  Following submission of consent, you will receive a link to book into one of the community clinics.

Vaccinations offered:

Flu (Nasal Spray or Injection) – Flu Vaccination Form

Given to ages 4 – 16 (Annually between Sept – Dec) Annual Flu Vaccination – Hampshire – Parents: Health for Kids

Human Papillomavirus (known as HPV) – HPV Vaccination Form

Given from age 12 (Jan – Aug) HPV (Human Papillomavirus) – Hampshire – Parents: Health for Kids

Tetanus, Diphtheria and Polio (known as Td/IPV) and Meningitis ACWY – Teenage Booster Vaccination Form

Given from age 14 (Jan – Aug) Teenage Booster (Tetanus, Diphtheria, Polio, Meningitis) – Hampshire – Parents: Health for Kids

Measles, Mumps and Rubella (known as MMR) – MMR Vaccination Form

2 doses routinely given at the GP surgery prior to starting school. The Immunisation Service offers the MMR vaccine to children that are missing any doses. MMR (Measles, Mumps, Rubella) – Hampshire – Parents: Health for Kids
