Your child is entitled to the same 5-19 Community Public Health Nursing Service as those attending formal education settings.

Our service is community-based rather than school-restricted. We can provide support through home visits or within local community settings. Support is collaborative and non-judgemental. Advice is tailored to meet the individual needs of child, young person, and/or family.

Growth and Development Reviews: We offer assessments to monitor your child’s physical growth in line with the Healthy Child Programme.

Health and Lifestyle Advocacy: Our team can provide evidence-based advice to help children and families implement healthier lifestyle choices.

Emotional Wellbeing Support: We can offer early intervention for and challenges with including anxiety, low mood, and self-confidence or anger management.

Targeted Guidance and Signposting: We provide specialist advice and referrals (signposting) for practical concerns such as sleep hygiene, toileting issues, and behavioural challenges.

We will not pass on anything you say to anyone else, unless you or someone else is at significant risk of harm. If we do need to share anything you have told us, we will always try to discuss this with you first.

For our safety and yours, any support will be recorded on electronic records that can be seen by other healthcare professionals who all follow the same confidentiality rules.

Immunisations

As a family engaged in home learning, your child remains fully eligible for the UK Universal Immunisation Schedule. The team offers community clinics and can tailor the delivery of immunisations to meet your child’s specific needs or requirements.

Contact us

ChatHealth is a confidential text support service for parents, carers, families and young people in Hampshire. Trained local nurses will be on the other side responding to your messages and are experienced in a wide range of health and wellbeing concerns you may have. As well as giving advice, the team can signpost to appropriate services.

Some things can take a little longer to work through. Complete the referral form into the Public Health Nursing Service for specific issues such as your child’s emotional wellbeing, growth concerns, fussy eating or ways to increase physical activity and reduce portion sizes.
