Leicestershire County Council Commission Healthy Together, Leicestershire Partnership (NHS) Trust to deliver the 0-11 Healthy Child Programme (Health Visiting/School Nursing) within Leicestershire and Rutland.

To ensure commissioned services are the best they can be, Leicestershire County Council regularly check feedback from service users. This questionnaire has been put together for you to be able to anonymously give your feedback on your experience. As this questionnaire is anonymous, Leicestershire County Council will not have any of your details saved, just your answers.
